Rapuora Mobile Nurses

 

The Rapuora Mobile Nursing service will support you to understand and manage your health.  We can work with you if you already have, or if you are at risk of getting heart disease, diabetes, or respiratory disease, and you are over 18 years old.

 

What can you expect from Rapuora?

We will visit you at home and discuss your health needs with you.

A health care plan will be developed with you. The plan will include information about your health, what you can do to support yourself and what other services you can access that may be helpful.  We will liaise with other health professionals as required and also help you to monitor your health.  You can expect Rapuora to call on a regular basis to support you to monitor your health and make sure the plan is working out.

Rapuora run health education seminars and other activities that you will be able to attend. You will also be welcome at other activities run by Te Puawaitanga. For information about activities that are coming up you can discuss with Rapuora, or contact us.

 

How can you enrol with Rapuora?

You or your whānau can enrol by ringing us directly,  through your GP, hospital or other health and community services can also refer you.

Healthy Day at the Pa

 

On the last Friday of the month at the Tuahiwi Marae, speakers provide health information and 

whakawhanaungatanga, followed by a shared lunch.
